Go to Columbus Public Health’s Office of Vital Statistics. The office is at 240 Parsons Ave, Columbus, OH 43215, and it issues certified copies of Ohio birth certificates for anyone born in the state.

This office can provide Ohio birth records from 1908 to the present, no matter which Ohio county the birth happened in. You can request a copy in person during posted weekday hours, by mail, or through the state’s online ordering system; a per‑copy fee applies.

If you’re requesting a newborn’s certificate, it becomes available after the hospital sends the record to the State of Ohio system. That usually takes a couple of weeks. For births that happened before 1908 in Ohio, contact the probate court in the county where the birth occurred.
